GitHub can render yaml issue templates as submission forms, which makes them slightly more user-friendly when rendered on GitHub. This replaces the current source submission.md template with a .yml one.
This template includes additional content to reflect the following:
Repo name
Repo site url
Repo description
Funding sources (if known)
Rationale for inclusion in the NDE:
Status (blocked, ready for review, etc. if blocked, explain why)
Task list for source integration
