Feedback on the new SLTT content added to the Cyber domain in alpha 2 (#215)

Delete element nc:Entity from type cyber:CyberInsuranceClaimType
Replace element cyber:BreachNotificationMethodCode in cyber:IncidentComplianceDriverType with cyber:BreachNotificationMethod

Changes applied to niem-5.1beta1.

Additional changes:

Renamed element CyberIndicator and its type as CyberIndicatorPattern to avoid conflicts with the NIEM Indicator representation term.
Changed the type of element UnattributedCyberIntrusionAnswerIndicator from nc:TextType to niem-xs:boolean.
Updated definitions for cyber:BreachAccessedQuantityCategoryCodeSimpleType codes Potential and Confirmed.
