Merge scr:PersonNameAugmentation and scr:PersonNameTextAugmentation #77

Screening has two augmentations defined for nc:PersonNameType. Recommend merging these two augmentations into one, keeping the scr:PersonNameAugmentation name.

The NBAC Harmonization Workgroup recommends merging these augmentations.

See also #96 for a proposal to move nc:EffectiveDate from the Screening augmentation directly to nc:PersonNameType.
