Redo ontologies as JSON so that we can incorporate hierarchical information which isn't currently supported by FSH #30

HPO, Mondo and NCIt all have hierarchical information which should be incorporated into our local representations of those terminologies. Doing so is necessary for taking advantage of FHIR's above/below type queries. While we do plan to offload the management of those terminologies elsewhere, there are current limitations with some of our development workflows that require that we maintain some portion of those terminologies within our model. This is expected to be a relatively short term requirement.

Sushi supports hierarchical codesystems since quite some time ago.
