added options for new indicator and other to prevent cases in which users could get stuck

@andybeet @sgaichas @jcaracappa1 i've added additional options for Indicator Name (New Indicator and Other) to prevent users from getting stuck in the catalog submission form. the google form has been revised with better instructions for submitting edits. the issue templates for editing pages are now properly linked in the google form.
