Modernize interface for cadn30, maxbyt, numtbd, igetntbi, elemdx, and parts of restd and stseq.

Part of #252

Combines cmpia, nummtb, and stseq into a single C file.

Looking at the diffs, there are some places where your name is 'J Ator' or 'J.Ator', I'll leave it to you if you want to search/replace those.
