release 1.4.0 #298

@Hang-Lei-NOAA @edwardhartnett could we do a release in the next day or two? I'd like to have a release with support for ip v5 (i.e., the consolidated sp+ip library) so we can ship ip v5 with the next spack-stack release.

I support to do a release based on previous changes in processing the large files. We can leave the new index to the next release.

Yes, but give me until next week. I am still doing testing for the 2 GB features. Everything looks good so far...
