Pre-assessment page added to setup wizard #73

Added UI for pre-assessment in setup wizard. After discussion with Kristina we decided that in a pre-visit context a user would potentially want to setup multiple assessments.

Have added ability to tie assessments to a piece of equipment.

Added ability to tag a contact with an assessment.

Potentially more data to be added lata for assessments.

Non-wizard pages had a couple pieces commented out and are a WIP.

has content from PR for contacts db will rebase after that has been merged
