Fix missing end of code block, code example consistencies, misspellings #25

The end of the Azure example was missing the triple backtick, causing the remainder of the README text to be formatted as code.

For consistency with the other examples, the prompt character '$' was added to the Azure example.

The word 'Existing' was also misspelled twice.

Hi @mitchpaulus. Thanks for pointing this out. This was clearly a typo on my behalf and probably a result of using vim to edit markdown. =) I know this is a minor doc related PR, but we request that you accept the contribution policy before we can merge.
