Patient records: Edit Patient Detail Form #207

Issue: Default Patient Info Under "Edit Patient Details" form doesn't match the current patient info e.g. BS2 card for this patient currently is yes but in the form it's no. affects all dropdown/date picker fields Similar to this issue: https://github.com/NUS-Project-SaBai/FrontEnd/issues/187

Where: patient records -> edit patient details

Expected: Should match the current patient details to avoid confusion
