Add issue template to this repository #35

Previous issues are filed but lacks of some key information such as running environment, running command&parameters. We'd better put a template like other projects such as spark-rapids :

Please provide a list of steps or a code sample to reproduce the issue.
Avoid posting private or sensitive data.

**Expected behavior**
A clear and concise description of what you expected to happen.

 - Environment location: [Standalone, YARN, Kubernetes, Cloud(specify cloud provider)]
 - Spark configuration settings related to the issue

Still not seeing the "Features" section, my permissions are: image

I'm wondering if it requires NVIDIA(organization level) admin to set it.

@wjxiz1992 I mean I have added the bug templates. So Do you want the permissions?

Let's hold on this issue. Since we're going to have another repo for spark & xgboost examples.
