Question improvements #274

This branch contains improvements for displayed questions (specifically improvements for fullscreen questions without the cursor shown) including:

  1. Removing the "submit" and "clear" buttons when the cursor isn't shown for questions (i.e. key-based responses)
  2. Vertical and horizontal centering of question prompt/options
  3. Disabling first person manipulator camera view control when displaying questions without a cursor
