Change signifier/field name from "Verified" to "Field Verified"

Add Unable to Locate to list
Remove/change N/A to No - Please Explain or Yes - Please Explain

We'll have to change the domain on the database side. I'm not sure if you can change a domain with data already in the column. This one will take a little finesse.
