Rtree fix #49

Includes fix so that latest version of rtree installs correctly. Also includes an example .env file so it is obvious where in the directory structure it should live, as well as some modifications to the README.

Current fix for rtree requires a sudo password. This might need to be a temporary fix. Thoughts?

put this on hold, we might containerize this repo just like how we did for facilities database we will use sptkl/docker-geosupport as base image In this case lib-spatialindex will be baked in
