Add City Record Online inputs to Hearing form #814

Integrating with City Record Online to send official hearing notices will be a huge win for user adoption of our app. The City Record Online requires a few questions that we don't currently capture in our form.

There are three questions about event accessibility that we need to add. They're highlighted in bright yellow in this spreadsheet. A couple questions:

  1. The Accessibility Contact Deadline -- do we want this to a be a date-time-picker? Do we think users will want to include a time on this or just a date? ANSWER: full date-time-picker. "What is your deadline for people reaching out for accessibility requests?"
  2. For the final email, do we want to do an HTML mailto link where we just have all the info set up for them in the body of the email OR do we want to actually send the email for them? <-- need to research how to do this ANSWER: Better to just send email for them.
  3. We'll have a radio button option on whether a user wants to submit city record info on our app. Once a user has selected this radio button, which of these fields do we want to make required? ANSWER: All are required.
