Headings made active and consistent in "protocol" list. In particular 9.3.4 Describe Patient, Exposure/Intervention, Comparator, Outcome, Time (PECOT) elements in text Change heading to Describe PECOT Elements in Text 9.4.6 Incorporate fairness and debiasing 9.5.2 Fill out methodology checklist 9.5.3 Review evidence quality

Also, PICOT replaced with PECOT
