A similar format as adopted with the main FMS (https://forests.nfa.go.ug/) - see https://www.figma.com/file/TknVBoMX7vWxnhAvIbRGHC/Long-forms could be used.

@ygoex - what do we need to move this forward as it seems the form can be created from the detail set out in #19 - would you like us to provide the sections / subsections for the form as per https://www.figma.com/file/TknVBoMX7vWxnhAvIbRGHC/Long-forms ?
