New FMAP - CFR entity

Add new CFR level fields defined in the FMAP structure spreadsheet to the CFR entity.

• [ ] Create a custom field for programs/activities
• [ ] Add a custom Program Tab field formatter plugin for Field Groups so that the tabs defined on the CFR can be used to build the Program/activity and sub-program dropdowns on the management task and budget task forms. See #299 and #300

As per my comment for #310 I think we can push this to production but potentially expect further changes when the NFA Forest Management Planning team are showcased it once we have some new data entered from the new plans they are developing using this format.
