Here is the list of sites for which there was this problem. It's quite long. The problem is that when the migration form was enabled something went wrong and a table wasn't created. (This was done when we were having capacity issues last week, so might be related to that). I've ensured this is fixed, but it means there's a number of sites that will need re-checking.
