H, im finding that LAV+DXVA2 will occasionally produce rendering glitches with H264/H265 content when the memory speeds drop down to idle during playback due to minimal activity. May be an issue with the current version of LAV and 40 series cards or Nvidia's drivers.
Edit: issue is not limited to LAV and may be an isolated issue on my hardware, still looking for a solution.
