Start on new incidental observations form #151

I tried using fpr_import_hab_con and subbed in the path of the .xlsm below but I ran into a bunch of errors. Will try to get it figured out next time so you can see the diff. But right now it's just a template I got from the website here. And I filled out the first row with test data to make guessing the types easier.
