match pscis submission input field values exactly in form inputs

An example of an issue is the capitalization of the havitat values in the form but not in the submission template.

we want to minimize what we do by hand so we can either use a populated spreadsheet table with all possible values added to the project or use a csv (see screenshot for that option). If we can use the same csv for all the fields and of course build it with R that is better than mouse work

form input fields that don't match up exactly with pscis spreadsheet:

• crossing type (options should be: 'Closed Bottom Structure', 'Open Bottom Structure', and 'Other')
• crossing subtype (options should be: 'Bridge', 'Pipe Arch', 'Wood Box Culvert', 'Round Culvert', 'Oval Culvert', 'Concrete Box', and 'Ford')
• habitat value (options should be: 'Low', 'Medium', and 'High')
