Tweak all layers to include multi-line comments for fields that contain comments (or text even)

Its a check box in the forms and makes it much easier to read. An example is historic pscis assessment and hab con comments. If our files are named the same our qlr file should transfer this from one project to another. Layers to apply to comments:

• [ ] form_monitoring

Issues arise when we search by attribute in Q and there are too many fields for screen. There is no scroll bar and it’s useless.
