Move edit ability level out of Add Note section #49

On ActivitySelect the option to change someone's ability level needs to move to a new modal called Edit Info. Currently it's hidden with the Add Note button.

Visually speaking, the Add Note and Edit Info modals should be consistent with the Add Visitor modal from FamilySelect.

Also add options to edit the following: FirstName LastName NickName DOB
