Remove 3rd party (lambda) service from app creation flow #18

Before publishing 3.0 this should be removed so the app creation commands go direct to the nexmo API, this means fixing the CORS issue

replaced the lambda function with a local backed within node red in d170f3baddba0f60520753a5472b7c251d14e1f3 CORS is not an issue anylonger as everything is on the same host
