CUDA tidy

[casperdcl]

• better CUDA error handling
• code de-deuplication
• better CUDA linking
• fix some compiler warnings
• remove unneeded variables
• use new LOG_FORMAT
• put > stream[%d] messages on one line
• use cmake_cuda for building
• move metadata setup.py -> setup.cfg
• add basic tests
