Adds code coverage and a badge using Istanbul and Coveralls. Right now we're around 70-80%, but the goal (obviously) is to improve that. :)

Merging without a +1 because I'm the only active person on this repo now (other than @mvayngrib), and I think this kind of housekeeping is very important to unblock other contributors.
