Missing Form Labels in Code Editor

In the code editor section of the application, form labels are missing, causing accessibility issues and making it difficult for users relying on screen readers to navigate and understand the content

Steps to reproduce

1. Access the code editor section of the application.
2. Navigate to any form input field (e.g., text input, checkbox, dropdown).
3. Attempt to locate the corresponding form label for the input field.

Expected Result:

• 0 Errors in WAVE evaluation tool

Actual Result:

• 3 Errors for missing form labels
