Step 3 - guiding questions - conditionals #97

Each time we ask them if something is hazardous, it should followup by asking what the hazard is if they answered yes. This doesn't always happen.

To simplify, let's have an open text box after each of these questions, and change the yes/no/I don't know to "Yes (fill in the hazards below)" / "No (fill in evidence of no hazard below)" / "I don't know".

To just have a text box, make a question that is " " (just a space). Can only have one per section. BUT could do multiple spaces maybe, see if that will work for a unique identifier?
