Discord allows duplicate positions in the channel list #58

Since there can be multiple channels with the same position, this causes channels to vanish in the channel picker. Use the type + position to add them to the array and hope discord doesn't allow the same type to also share positions.

Hasn't been an issue since with anyone. Reopen if it happens again.
