cancel his job
turn on an alert
time to sleep

Appearing to be having same issue. Noticed it seems to happen around ladders/fire poles and airflow tiles. Also appears to be dropping into natural tiles as that tile was put in via sandbox with nothing behind it.

Watched it happen over the course of 1-2 hours on normal speed. Seems to occur under one or more of the following conditions. 1) Dupe is unable to breath, 2) near/on ladder (or fire pole), 3) called away on a task (downtime, etc)
