Add left-hand table of contents rendering to Markdown versions of Specification and Implementation Notes #604

We had this is 1.0 through ReSpec. Other Markdown/Jekyll specs like IIIF Presentation API do this. Issue raised in https://github.com/OCFL/spec/pull/596#issuecomment-1133745563
