Fix NPE #24

Redesign for results where LOINC is known but exception occurs before observation is parsed.

Once this bug fix is merged, I think it will be a good time to release version 1.0.0 of the library. In the next couple of weeks, I'll follow up with additional work on panels, but this is a natural stopping point for a deployed artifact.

I went ahead and updated to the latest Phenol version on master to ensure there were no breaking changes. I've requested a tagged release of this which we'll need before we can release fhir2hpo.
