Provided Uptime Monitoring Dashboard for Monitor My Watershed #729

Summary

Following the migration to AWS for hosting under release 0.12.0, we deployed an uptime monitoring service to track and notify the development team of site outages. This information has been primarily kept internal. This issue is intended to make some of that information available via a public dashboard. The third party monitor service we use, makes available a number of widgets to share this monitoring information publicly.
