A clinical history taking is "an interview in which a clinician elicits a
clinical history from a patient or from a third party who is reporting on
behalf of the patient." [1]
A planned process is "a processual entity that realizes a plan which is the
concretization of a plan specification." [2]
I'm having trouble imagining clinical history takings that do not realize such
a plan. Putting 'clinical history taking' under 'planned process' would bring
it closer to other things it resembles, like 'informed consent process' [3].
The patient history questionnaires I'm working with are (or contain) plan
specifications [4].
