SourceName is not retrieved from CDM Source #704

Minor issue. If no source name provided, then this will stay an empty string.

Line 200 is missing assignment to sourceName. https://github.com/OHDSI/Achilles/blob/a9144b2f3119055b76dfe8e9d798654558127fe3/R/Achilles.R#L196-L201
