Expose Index and Matching cohortID #5

Need to present the index cohort id and matching population cohort id in the UI. Currently there is no way to get the generated cohort data without looking at CIRCE for the cohort definition list (and even then it's not straight forward to find it). In the summary of the study, there should be the cohort IDs visible.
