proposal details: no new table is added.

# new conventions:

OBSERVATION table: Use trial NCT identifier to identify a study. Use the following concepts to capture the following study events (see forum post here: http://forums.ohdsi.org/t/omop-cdm-and-clinical-trials/2109/7 )

(when links are copied they are changed by github platform for some reason)

@vojtechhuser I think this may be more of a THEMIS issue as it deals with conventions
