again we have this existing language "globally controlled". I am not a huge fan of this since we have a domain of payer and a vocabulary with some information that is expected to be there. I will alter this language to suggest the creation of custom concepts or community contribution if there is not one in the expected vocabulary to represent the payer in the native data.
