d_micro table no longer available #14

d_micro table was removed in v0.4 of mimic iv but it's still being used in the etl scripts

thanks @Esha-Singh1999 , for pointing this out. In the initial Demo version of MIMIC IV which we are still using, it is still present but in the full MIMIC IV representation it does not show up anymore. As we have been running the ETL logic against the full MIMIC IV I would assume we do not experience major issues but we will follow up and remove d_micro from the scripts going forward.
