@toanong Please merge the bugfix branch's commits to master. I have made changes to readme file and added a new markdown version of the ETL description document. Also, we may remove the word version of ETL description file if that's redundant.

@toanong Made changes to sql for the enrollment table to pull information form observation_period as opposed to payer_plan_period since PEDSnet partners dont have insurance information as yet. This has been documented in the ETL description document. In the future is a site does have the insurance information then they should pull from the payer_plan_period table (i.e. record insurance based enrollment). Also made several minor changes to read me file.
