Add terms for missing stage groups to Cancer Modifier #567

Stage IVE exists in the LOINC vocabulary, but has been destandardized (as part of #516) and needs to be mapped to a Cancer Modifier term. None currently exists.

Highly likely that this term is valid as it also appears as a SNOMED term

SNOMED terms Stage 1m, A1, A2, B3, C1, C2, C3 are not in Cancer Modifier but also exist in Nebraska Lexicon (as non-standard, mapped to these SNOMED terms)

Nebraska Lexicon term Stage 6 has close match WR Stage 6 in SNOMED, but there is no term in Cancer Modifier.

These terms are likely valid as they appear in both SNOMED and Nebraska Lexicon.

These terms have been destandardized as part of #517
