Should there be a "cancer spread" umbrella term above metastasis and extension/invasion? #608

Within the Cancer Modifier vocabulary, there are no parent concepts above:

Metastasis and Invasion

Additionally, there is no umbrella "extension" concept.

Proposal:

Create a "cancer spread" umbrella concept with child concepts of "metastasis", "invasion", and "extension"
