Improper standard concept mapping for Arthropathy #512

For these ICD9CM concept ids:

44836160 711.19 Arthropathy associated with Reiter's disease and nonspecific urethritis, multiple sites
44837325 711.89 Arthropathy associated with other infectious and parasitic diseases, multiple sites
44824491 711.59 Arthropathy associated with other viral diseases, multiple sites
44834967 711.49 Arthropathy associated with other bacterial diseases, multiple sites

The standard concept mapping is to both of these:

4116450 Secondary multiple arthrosis
76498 Arthropathy associated with bacterial disease

We believe only the second one should be an appropriate mapping, whereas the first one is not a good fit. Can it be removed?

Hello @alondhe! The actual mapping for these ICD9CM concepts is following: icd9cm_concept_id icd9cm_concept_code icd9cm_concept_name snomed_concept_id snomed_concept_name
44836160 711.19 Arthropathy associated with Reiter's disease and nonspecific urethritis, multiple sites 78357 Reactive arthritis triad
44836160 711.19 Arthropathy associated with Reiter's disease and nonspecific urethritis, multiple sites 4116450 Secondary multiple arthrosis
44837325 711.89 Arthropathy associated with other infectious and parasitic diseases, multiple sites 76777 Arthropathy associated with infection
44837325 711.89 Arthropathy associated with other infectious and parasitic diseases, multiple sites 4116450 Secondary multiple arthrosis
44824491 711.59 Arthropathy associated with other viral diseases, multiple sites 77348 Arthropathy associated with viral disease
44824491 711.59 Arthropathy associated with other viral diseases, multiple sites 4116450 Secondary multiple arthrosis
44834967 711.49 Arthropathy associated with other bacterial diseases, multiple sites 76498 Arthropathy associated with bacterial disease
44834967 711.49 Arthropathy associated with other bacterial diseases, multiple sites 4116450 Secondary multiple arthrosis

You are right, they all have 1-to-many mapping and sometimes mapping to more than one standard concept can look shady. We try to avoid it. However, in this case, we can't capture the whole meaning of the ICD9CM concept by only one SNOMED's (It is not clear that multiple joints are involved in the pathological process from the concepts like Arthropathy associated with ... as well as we can't show the etiology using only 4116450 Secondary multiple arthrosis concept) That is why I don't think that it needs improvement.

@alondhe I think it relates to the general problem discussed here: https://forums.ohdsi.org/t/condition-mapping-improvement-using-snomed-extension-proposal/3880

Can we close the issue?

Hi @alondhe just checking again, can we close the issue now?

alondhe commented 1 year ago

Hi, yes, you can close it. I suppose it's part of the larger need for SNOMED Extension
