Mapping suggestion for laterality of primary open-angle glaucoma from ICD10 to SNOMED #650

Hi,

I have submitted an abstract on primary open-angle glaucoma (POAG) phenotype algorithms to the OHDSI 2022 Symposium, and part of what I am trying to explore within my analyses is how to more accurately map laterality from non-standard vocabularies, such as ICD-10, to standard SNOMED concepts.

In the ICD-10 vocabulary, laterality is coded within the alphanumerical code itself as a specific character after the decimal point in the code. Universally:

A value of "1" indicates a laterality value of right eye; A value of "2" indicates a laterality value of left eye; A value of "3" indicates a laterality value of bilateral (both) eyes; and a value of "4" indicates a laterality value of unspecified eye.

In the case of POAG, the laterality is derived from the third digit after the decimal place.

Currently, there are zero descendant record counts for both right and left-eye POAG using SNOMED codes:

And there aren't any current bilateral or unspecified mappings that I saw or am aware of, so if it is possible to create a new concept for these, I would also recommend that.

I would recommend the following mappings from ICD-10 codes to SNOMED, per the above, where an "X" is a wildcard indicator and can take any alphanumeric value:

  1. Any ICD-10 code in the pattern of: "H40.XX1X" maps to SNOMED concept ID 761205.
  2. Any ICD-10 code in the pattern of: "H40.XX2X" maps to SNOMED concept ID 761148
  3. Any ICD-10 code in the pattern of: "H40.XX3X" maps to a new SNOMED concept ID, which should have a concept name of "Primary open angle glaucoma of bilateral (both) eyes"
  4. Any ICD-10 code in the pattern of: "H40.XX4X" maps to a new SNOMED concept ID, which should have a concept name of "Primary open angle glaucoma of unspecified eye"
