Adds stg models for careplans, claims_transactions, claims, devices, and encounters (#11 , #12, #13, #15, #16)
Updates column names in person stg model to remove unnecessary person_ prefix from some columns. Only need tablename prefix for potentially ambiguous column names (i.e., used in other source tables)
Adds initial style guide (#8)
Adds SQLFluff (#7) and related documentation
Creates macro for lowercasing column names
