This PR introduces a script to generate DBT docs and tests from the CDM definition (from HTML documentation).

Includes:

This output works in dbt docs.

Some data in the OMOP docs are wrong (upstream of this PR, will see if fixable at some point)
Some odd/atypical characters used - e.g. ’ (U+2019 RIGHT SINGLE QUOTATION MARK) - but is handled fine by dbt docs
