docs: improve / customise default `overview` page of dbt docs #86

At present the default dbt docs page for this project is the vanilla dbt one - I think this is does not massively help newcomers to the project.

I think there is scope to have documentation on what dbt is, alongside OMOP/Synthea and explain how/why the schemas are laid out the way they are etc.
