Treat observations without encounters as "Home" #208

To resolve an issue we are seeing in production, we will begin treating all observations without encounters as if they are home observations. This may be a temporary solution, as it could have impacts on other institutions.

This was done and pushed to production, but we are planning on reverting it once the real fix from COACH is in place. Leaving open for tracking.
