Fix a deadlock in binary channel on cleanup, reduce contention due to locks on high server load, remove m_monitoredItems

• Fork from #2714
• Also remove unused m_monitoredItems: Items were being added with a numeric key and never removed, causing a memory leak every time a session was reconnected
